- STATEMENT -

As part of our professional relationship you entrust and I record personal information about you. This ensures I can provide the best possible treatment and care for you.

In line with EU General Data Protection Regulations (GDPR) I am legally obliged to keep your information secure, not share it with anyone without your permission, allow you to access your information at any time, to correct or update information if appropriate, delete it after a suitable time and to inform you of any significant breaches.

When you book an appointment and agree to a course of treatment I will require to record your name, address, telephone number and email. This information is kept in a secure, password protected contact list.

During the course of a consultation I will ask you further questions about your medical history, personal like and dislikes, general health and well-being - this sensitive information is not stored online and will never be shared with a third party. This information is kept locked and secure.

 

In line with GDPR guidance I only collect data from you that is relevant to your treatment, best practice and personal care. I may need to contact you via phone, email or mail about a booking in response to an enquiry. You may request to see, amend or have your data deleted at any time.
